Mentoring Forms

The Women’s Mentoring Program uses two forms to help mentors and mentees plan for their meetings and keep on track with their plans.

The Mentee Action Plan (MAP) is for mentors and mentees to work out their overall goals for the program. The MAP may be updated as the program progresses. If you’d prefer to have the MAP as an editable Word document, click here instead.

The Immediate Goals and Objectives (I GO) plan should be filled out by the mentee and sent to the mentor at least three days before each mentee-mentor meeting. The I GO form helps to structure these meetings by laying out objectives, challenges and opportunities.
